Target Housing’s South Yorkshire Care Service Shortlisted for Prestigious Care Award

It’s our pleasure to announce that Target Housing’s South Yorkshire Care service was recently shortlisted for the prestigious Housing with Care Award by the Great British Care Awards!

The award celebrates exceptional services provided for those in supported housing, with nominees demonstrating their ability to meet complex care and support needs while helping people to live as independently as possible.

Our South Yorkshire Care service has been shortlisted in recognition of the support it provides to people with complex needs, many of whom have previously struggled to access traditional care services.

Our South Yorkshire Care Service

SYC was developed to offer a more flexible and person-centred approach, looking beyond the barriers someone may face and instead focusing on the support they need to build a more stable and independent life.

South Yorkshire Care was created in response to a clear gap in support for people with complex needs. Many individuals were struggling to access the care, services and housing they needed, making it harder for them to move forward and build stability.

For some people, this meant essential care needs were left unmet, leading to poorer health outcomes, housing instability and repeated periods of crisis.

The service works closely with individuals to understand their needs, experiences and goals, creating tailored support plans that can include help with medication, accessing health services, developing everyday living skills and maintaining a safe and suitable home.

How the service helps

Individuals like David face barriers to accessing the care they need to achieve the independence and opportunities they deserve. In David’s case, a decline in his mental health meant he could not return to his previous accommodation, and other providers were unable to offer suitable support. Despite being ready for discharge from an acute mental health ward, no other housing providers would accept him.

Where previous services had struggled to meet David’s needs, South Yorkshire Care focused on what could be achieved with the right support in place.

After further struggles with his symptoms, David was assessed by a housing officer and offered self-contained accommodation, providing him with his own space to manage his symptoms whilst having access to specialist health and substance services.

Four years later, David has settled into his community and engages in the activities of daily living with the continued support of SYC and CMHT. David’s story highlights the importance of specialist care services for those with complex needs and is a prime example of how the right support can help previously overlooked individuals achieve their potential.

From then to now

Since it was established 4 years ago, SYC continues to provide flexible needs-led services with high standards of support and assistance.

From Target Housing becoming a registered care provider with the CQC to building a dedicated and growing team, the service has continued to develop while keeping person-centred care at the heart of everything it does.

Being shortlisted for the Housing with Care Award is a fantastic recognition of the hard work and commitment of everyone involved in the service.

Whatever the outcome at the Great British Care Awards ceremony, reaching the Yorkshire & Humber regional final is an achievement to be proud of and reflects the positive difference South Yorkshire Care continues to make in the lives of the people it supports.
