Target Housing is proud to announce the launch of St Barts, a new specialist supported living service designed to support individuals with enduring mental health conditions, learning disabilities and/or autism, alongside severe multiple disadvantage.
St Barts has been developed as a 10-unit service for individuals whose needs have often not been met by previous provision. Many people supported by the service will have experienced multiple placement breakdowns, prolonged hospital stays, or discharge into the community without the right level of ongoing support. The service has been designed to respond to these challenges through a highly personalised, trauma-informed approach.
Residents at St Barts may have experienced significant and complex trauma, which can result in behaviours that challenge, including high-risk behaviours towards themselves and/or others. In response, the service provides intensive, wraparound support delivered by a dedicated on-site team consisting of Recovery Case Workers, a Recovery Team Leader and a Service Manager.

Support at St Barts includes crisis intervention, co-ordination of care and support, housing management, and robust risk management. The team works closely with community link workers and professionals across health, mental health, and drugs and alcohol services, ensuring that individuals have access to the right tools and support to aid their recovery.
A key feature of the St Barts service is its person-centred model. Following careful consideration of the impact of trauma and the importance of flexibility in recovery, there is currently no fixed end date to the support provided. Residents are encouraged to move towards independence at their own pace, with the overarching aim of helping individuals sustain a home and improve their overall wellbeing.
Due to the complexity of need within the cohort, St Barts is staffed 24 hours a day, making it the first service of its kind operated by Target Housing. This round-the-clock support ensures residents can access assistance whenever they need it, offering reassurance, safety, and continuity of care.
Referrals into the service are made directly through commissioning leads in mental health and learning disability/autism pathways. Each referral is discussed at panel before a full assessment is completed by the team, followed by structured transition work for individuals moving into the service.

The launch of St Barts marks an exciting and important step forward for Target Housing, reflecting the organisation’s continued commitment to developing innovative services that meet the needs of people facing the most complex barriers to stable housing and recovery.
